Please choose the type of request you are submitting: Typical response times for low and medium priority issues will be within 24 hours from the time we receive the request.
For any request received that is Mission Critical (high priority), someone from our Service Team will be in contact within 2 hours from when the request was submitted.
To ensure expedited service, please also call (303) 650-7500 and ask to be connected to the Service department once you've completed and successfully submitted this request form.
Please describe the issue you're experiencing in as much detail as possible, and include the name of the product or system you're referring to.
Proposed Date for Service Please list the unit, floor, rooms, or areas that are experiencing the issue, or impacted by the issue.
Please provide Control Panel information. Be sure to include the Address and Input/Output descriptions (if applicable)
Please provide Access Control information below. Be sure to include the Door Name(s), Type of Door and Equipment installed, for example: NDE, Mag Lock, Delayed Egress, Crash Bar, Electric Strike, etc. (if applicable)
Please provide Security Surveillance information here. Be sure to include the Camera Name(s), Type of Camera(s) or equipment installed, IP address, and MAC address (if applicable)
Please provide Security Intercom information here. Be sure to include the IP Address and Mac Address (if applicable)
Please provide us with any additional information that may be helpful for us to know regarding this change request.

Please give 48 hour notice to Beacon via email to cancel any testing requests. If rescheduling, we will require 48 hours before we can schedule a new testing date and time.
Requestor agrees that any lost time due to lack of communication to Beacon as it relates to the scheduling of services are subject to pay for Beacon's lost time and labor at their designated T&M rate.
